We’ve talked about the passing of Amendment 2, we’ve discussed the challenges the industry is facing, but what we haven’t discussed yet is how a patient obtains their medical cannabis license in the state of Missouri. The following is a broad scope of how to apply for your license.
Part I. Physician Recommendation and Patient Certification: |
A. Obtain your qualifying diagnosis records; these records can be from any licensed physician with the exception of mental health diagnosis records which much come from a board licensed psychiatrist.

B. Contact a clinic that is participating in the certification process to schedule your patient certification appointment. We recommend The Green Clinics, founded by Emily Branch. They provide statewide certifications as well as tele-health appointments for those patients that cannot travel to a physical clinic.

C. During your certification appointment, your doctor will complete the Physician Certification Form after reviewing your medical records and speaking with you about your medical/treatment history.

D. If you and your physician both agree that an extended possession amount (beyond four (4) ounces) is beneficial to your treatment, two doctors certifications are required.

E. Your physician certification must be no more than thirty (30) days old when you apply with Missouri Department of Health and Senior Services. |

Part II. Missouri Department of Health and Senior Services, Patient Application Process |
Section A) Answer these questions prior to applying… |
1) Do you want to cultivate your own cannabis? |
2) If you want to personally cultivate, does your current residential living situation allow for the legal cultivation of cannabis? Most rental properties will not allow cultivation. |
3) Do you want to authorize a primary caregiver to grow and purchase your medicine?

4) Do you have all of the required information documents to apply for your license?
|
5) Once you have determined that you have everything necessary to complete your application, proceed to the MMJ portal registration portion prior to your application. You must first register on the portal and confirm your email address prior to completing your application. |
6) Once you have registered and confirmed your email address, you can proceed to the MMJ portal application.

7) While you are completing your patient license application, you must determine if you as a patient want personal cultivation AND if you are going to cultivate yourself OR if you will enter into an agreement with a caregiver to cultivate your medicine.

Section B) Patient Self Cultivation & Caregiver Cultivation |
1) According to 19 CSR 30-95.030 Qualifying Patient / Primary Caregiver the following must be included: |
2) The address of the facility in which the qualifying patient will cultivate marijuana; |
3) A description of the security arrangements and processes that will be used to restrict access to only qualifying patients and their licensed caregiver; |
4) The name and Patient License Number or Caregiver License Number, if applicable, of one other qualifying patient or primary caregiver with whom the cultivating facility will be shared;

5) A statement affirming the applicant’s agreement to immediately make available access to the patient cultivation facility upon request from the department.

Part III. Submitting Application |
1) Once all of the pages of the electronic application have been saved, take time to review everything before submitting |
2) Ensure you have properly copied all of the information from the physician certification form exactly as its written by the physician. |
3) Ensure you have BOTH pages of your physician certification uploaded. |
4) Ensure all photos/scans of supporting documents are clear and legible. |
5) Save and submit. |
6) You will receive email confirmation that your application is submitted and pending approval. |
7) Within 7 business days, you should receive your approval or denial.

8) You can check the status of your application anytime by logging back into your patient portal. |
9) Once you have received your approval, its time to download your patient ID card.
